Response types

We offer a variety of response types to accommodate different checklist questions. Note that all questions are mandatory i.e., users must answer all questions before they can submit the check.

Yes/No

For questions that only have binary options.

“Are there 3 boxes?” ”Does the premises have a fire extinguisher?

Number

For reporting of actual quantities.

“How many are there?”

Photo (.jpeg or .png; one photo only up to 5mb)

For collecting photo to prove a specific check has been done.

“Submit a photo of the fire exit sign.”

Checkbox

For steps in a process or instructions that need to be carried out.

“Charge the battery.”

Dropdown

For questions that have multiple custom options.

“Select the type of premises.”

Textbox

For submitting of free text.

“Name of the premises.”

Date and time

For submitting of date, time, or both.

“Date of audit.”

Logic & Scoring

Logic and scoring features are also available for selected response types.

Logic

Only the ‘Yes/No’ response type supports this feature today. A custom question will be revealed when the user selects either ‘Yes’ or ‘No’. This question currently supports ‘Number’ response type only.

'Yes/No' response type

Scoring

Only ‘Yes/No’, ‘Checkbox’, and ‘Dropdown’ response types support this feature today. Agency admins who create the checklist can pre-assign scores to the each response. This supports checklists that need to compute a total score for compliance monitoring purposes.
